A formal doctor-patient relationship is established only when:

  • You complete and sign intake paperwork at our facility (this is when protected health information is collected)
  • You provide informed consent for treatment
  • Services are provided by Dr. Edgar Rodriguez or other licensed practitioners

Website inquiries do not establish a doctor-patient relationship and do not involve collection of protected health information.
